CARE MANAGEMENT
Care Management is a service in our office that provides a partner to work personally with our patients who have chronic conditions such as hypertension, diabetes or obesity.
Jamie Snow has been our RN Care Manager since the fall of 2023. She is here to educate our patients so they can become more effective participants in improving their health and answer any questions to help you live the best life possible.
